RE: 2019 Community Development Block Grant (CDBG) Contract amendment
Introduction:
The intent of this Agenda Item is to seek the City Council's authorization for the City Manager
to amend the Community Development Block Grant contract with IBN Sina Foundation to
include rental,telephone, and internet expenses for the clinic at 8455 9th Avenue.
The current contract only includes expenses for electricity, water, and natural gas.
The contract amount will remain at $5,000.
Background:
Resolution number 19-379 approved the $5,000 CDBG allocation to IBN Sina Foundation. The
Foundation provides free or low cost health care services for the uninsured, low income
individual and families in Port Arthur five days a week.
Budget Impact:
Community Development Block Grant program is the only funding source.
Recommendation:

RESOLUTION NUMBER
A RESOLUTION AUTHORIZING THE CITY MANAGER TO EXECUTE AN
AMENDMENT TO THE COMMUNITY DEVELOPMENT BLOCK GRANT
CONTRACT WITH IBN SINA FOUNDATION TO INCLUDE MONTLY RENTAL,
TELEPHONE,AND INTERNET EXPENSES FOR THE CLINIC AT 8455 9TH AVENUE.
THE CONTRACT AMOUNT WILL REMAIN AT $5,000.
WHEREAS, Resolution number 19-379 allocated $5,000 to IBN Sina Foundation to
provide free or low cost health care services for the uninsured, low income individual and
families in Port Arthur five days a week; and,
WHEREAS, IBN Sina Foundation requests to amend its Community Development
Block contract to cover the expenses for rent,telephone, and internet services along with the
monthly water, electricity, and natural gas expenses for the clinic (see Attachment"A"); and,
WHEREAS,the City Council now finds it desirable and appropriate to amend the
Community Development Block contract with IBN Sina Foundation to include monthly rental,
telephone, and internet expenses; and,
WHEREAS, the City Council is required to designate an official to sign all documents in
connection with the amendment; now,therefore,
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PORT
ARTHUR:
THAT the City Manager is authorized to execute an amendment to the contract with IBN
Sina Foundation for the proposal outlined herein.
THAT a copy of the caption of this Resolution be spread upon the minutes of the City
Council.

GRANT AWARD TO IBN SINA COMMUNITY CLINIC PORT ARTHUR
The Board and Management of Ibn Sina Foundation are very grateful to City of Port Arthur for
the grant of$5,000.
This grant is approved for utility expenses including electricity, water and gas. However, we
have some additional expenses including rent,telephone and internet for smooth operation of the
clinic.
We request you to allow Ibn Sina Community Clinic Port Arthur to use the grant for rent and
additional utility expenses. We are assuring you that the total expenses will not be charged over
$5,000 to City of Port Arthur.
We highly appreciate your support and look forward for your continued support to Ibn Sina
Community Clinic Port Arthur to serve the underinsured and indigent community.

low income individuals and families in Port Arthur.
CONTRACT TERM: 365 days
ORIGINAL PERFORMANCE STATEMENT: Community Development Block Grant
(CDBG) funds will reimburse for utility expenses: water, gas, and electricity. CDBG will only
pay the current monthly charges for the utilities. CDBG will not pay late fees or unpaid charges
from previous months. Receipts and copies of cancelled checks must be attached to
reimbursement statement.
NEW CONTRACT Performance statement including the AMENDMENT will be:
