A Disadvantaged Community Worksheet must be submitted in order to be considered for this funding option. This
worksheet involves the use of socioeconomic data to determine if the affected service area meets adjusted annual median
household income and affordability criteria.

Determining the adjusted annual median household income (AMHI) and average household size for the affected service area can be
achieved by:
• using the most recent decennial US Census data, or
• conducting a survey as outlined in the Socioeconomic Survey Guidelines (SRF-008a).
Please check the method below that you will use to determine the AMHI and average household size.

If your service area is not a city or town, you must provide a table that 1) identifies the number of household
