RECOMMENDATION
It is recommended that the City Council adopt Proposed Resolution No. 12153 authorizing the
City Manager to execute a contract between the City of Port Arthur and Texas Department of
Health to continue the STD/HtV program. This is a renewal contract.
BACKGROUND
Last year's contract ended 12/31/02. The renewal contract was held up in Austin and we did not
receive it until April 10, 2003; nevertheless, STD/HIV services continued. Payment for these
services wilt be retroactive to January 1, 2003.
With this grant award the City of Port Arthur and TDH have agreed to jointly provide for, conduct
and maintain programs to control and/or prevent the spread of STDs and HIV, and to provide
expanded outreach services to individuals of identified sub-groups who have tuberculosis (TB)
or who are at high risk of developing TB. The grant award of $44,719 is the same amount that
we received last year.
BUDGETARY/FISCAL EFFECT:
A total of $44,719 has been awarded for the program. This grant will provide for salaries, fringe
benefits and travel for one employee to conduct STD/HIV prevention and TB control activities.
STAFFING/EMPLOYEE EFFECT:
This grant funds one Public Health Specialist II position.

A RESOLUTION APPROVING THE FY 2003 CONTRACT
BETWEEN THE CITY OF PORT ARTHUR AND THE TEXAS
DEPARTMENT OF HEALTH TO CONTINUE THE STD/HIV
PROGRAM
WHEREAS, the FY 2002 contract between the City of Port Arthur and Texas
Department of Health to provide for STD/HIV services was approved on January 8, 2002,
Resolution No. 02-08; and,
WHEREAS, the City of Port Arthur has received a total grant award of $44.719 to
continue its efforts in detecting, treating, and preventing STDs in our community, as well
as to provide expanded outreach services to individuals of identified sub-groups who have
tuberculosis (TB) or who are at high risk of developing TB;
WHEREAS, the contract term is from January 1,2003 through December 31,2003;
now therefore,
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PORT ARTHUR:
THAT the Contract, a copy of which is attached hereto and made a part hereof as
Exhibit "A" is hereby approved; and,
THAT said Contract is an exact duplicate of a City Standard Form Contract except
for the changes described in the Certificate of Standard Form Contract, a copy of which
is attached hereto as Exhibit "B" and is hereby incorporated by reference, and said
changes are hereby approved; and,
THAT the City Manager of the City of Port Arthur and Ernestine Wade, RN, Director

of the City's Health Department are hereby authorized and directed to execute said
Contract on behalf of the City of Port Arthur; and,
THAT a copy of this Resolution be spread upon the Minutes of the City Council.

SECTION I. SCOPE OF WORK:
PERFORMING AGENCY shall conduct programs to control the spread of Sexually Transmitted
Diseases (STDs) through Prevention Counseling and Partner Elicitation. (PCPE) activities and
shall provide, or have provided, social/support and medical care services to reduce the spread of
human immunodeficiency vires/acquired immunodeficiency syndrome (HIV/ADS), and shall
provide expanded outreach services to individuals of identified sub-groups who have
tuberculosis (TB) or who are at high risk of developing TB throughout PERFORMING
AGENCY'S defined service area.
PERFORMING AGENCY shall comply with all applicable federal and state laws, roles,
regulations, standards, and guidelines in effect on the beginning date of this contract Attachment
unless amended. The following documents are incorporated by reference and made part of this
contract Attachment:

Control and Prevention (CDC);
· Recommendations for the Prevention and Management of Chlamydia Trachomatis
Infections 1993:08:06 (CDC);
· Policy Guidelines for Prevention and Management of Pelvic Inflammatory Disease (PI]))

Within thirty (30) days of receipt of an amended standard(s) or guideline(s), PERFORMING
AGENCY shall inform RECEIVING AGENCY Program, in writing, if it will not continue
performance under this Attachment in compliance with the amended standard(s) or guideline(s).
RECEIVING AGENCY may terminate the Attachment immediately or within a reasonable
period of time as determined by RECEIVING AGENCY.
PERFORMING AGENCY shall perform the following PCPE services:
Ensure that during clinic visits patients are offered information and .education about
HIV/STD infection. Information and education may be pamphle? and/or video programs
or HIV/STD messages given by health department staff;
Offer culturally competent and effective prevention counseling based on RECEIVING
AGENCY Program-approved model; and,
Use scannable forms or other comparable data collection systems supplied or approved
by RECEIVING AGENCY Program for those clinic patients who receive prevention
counseling.
PERFORMING AGENCY shall develop and implement an effective, trackable referral system
with written procedures by which HIV/STD reciprocal linkages are established with other
service providers (especially other RECEIVING AGENCY funded providers) for the purpose of
obtaining medical care and social/support services to meet the needs of clients.
PERFORMING AGENCY shall submit confidential reports of diagnosed cases of syphilis as
required to RECEIVING AGENCY Program in a manner and in a format provided by
RECEIVING AGENCY Program and described below in PERFORMANCE MEASURES.
PERFORMANCE MEASURES
The following performance measure(s) will be used to assess, in part, PERFORMING
AGENCY'S effectiveness in providing the services described in this contract Attachment,
without waiving the enforceability of any of the other terms of the contract.
PERFORMING AGENCY shall provide ninety percent (90%) of the 13 persons who are
diagnosed in PERFORMING AGENCY clinic as a case of Primary or Secondary
Syphilis with education, partner elicitation and partner notification, social/support and
medical care services. The persons who receive the services shall be those who live or
receive services in the following county(ies)/area: Jefferson.
Eighty percent (80%) of the current fiscal year impact and process objectives shall be met
as referenced by the 1999 Program Objectives and Indicators Report.

The following STD clinical services shall be provided:
A minimum of 90% of clients seeking STD diagnostic and/or treatment services
at public STD clinics during normal operating hours shall be medically served the
same day; and,
2. At least 99% of clients seeking STD diagnostic and/or treatment services in public
STD clinics are medically managed according to protocols in the "STD Clinical
Practice Guidelines" - 1991 (CDC) and "STD Treatment Guidelines" - 1998; 47
(No. RR-1) (CDC) (or most current version).
Reports and Narratives:
Narratives and objectives - Due senti-annually on the 20th day of July and
January.
STD Management Information System (STD MIS) data - Due weekly on Friday
by 5:00 p.m.
Congenital Syphilis Case Investigation and Infant Syphilis Control Record - Due
to RECEIVING AGENCY, Bureau of HIV and STD Prevention, thirty (30)
calendar days after the case is reported to the local health department.
SECTION II. SPECIAL PROVISIONS:
PERFORMING AGENCY shall authorize their staff to attend training, conferences, and
meetings for which funds were budgeted and approved by RECEIVING AGENCY Program.
PERFORMING AGENCY'S personnel shall be made available m staff a Rapid Intervention
Team (as referenced in the "Outbreak Response Plan" of the Comprehensive STD Prevention
Systems [CSPSI Announcement Number 99000 Federal Grant application dated October 2,
1998) in the event of a disease "Outbreak" as interpreted by RECEIVING AGENCY Program.
PERFORMING AGENCY'S personnel shall travel to other Texas jurisdictions that may not be
included in PERFORMING AGENCY'S Service Area or Region as requested by RECEIVING
AGENCY Program during this event.
PERFORMING AGENCY personnel shall submit all travel expenses (including
accommodations and meals), while in another Texas jurisdiction, to RECEIVING AGENCY
Program or the host jurisdiction. If PERFORMING AGENCY'S travel requests to RECEIVING
AGENCY Program exceed the amount budgeted, this contract Attachment may be amended to
cover allowable costs according to the PERFORMING AGENCY'S approved travel plan or, if

there is no approved travel plan, in accordance with the State's travel rates as set out for its
employees.
RECEW1NG AGENCY Program will supply PERFORMING AGENCY with the most current
version of the document entitled "Content of Ali)S-Related Written Materials, Pictorials,
Audiovisuals, Questionnaires, Survey Instruments, and Educational Sessions in Centers for
Disease Control Assistance Programs," and PERFORMING AGENCY shall provide
RECEIVING AGENCY Program with written acknowledgment of receipt and assurance of
compliance with these guidelines.
Due to the sensitive and highly personal nature of STD HlV/AEDS-related information, strict
adherence to the General Provisions, Confidentiality Article, is required,
Due to the specialization and job knowledge required for effective STD control programs, the
following minimum training is required of personnel employed under this contract Attachment:
Disease Intervention Specialist (DIS) shall successfully complete the following training
within six (6) months of employment:
a. STD Employee Development Guide;
b. Two (2) week Introduction to STD Intervention course;
c. Training to perform venipuncture; and,
d. PCPE training.
First-line supervisors shall have completed, within one (1) year of becoming a supervisor,
all training requirements of a DIS, plus:
a. Attended the Principles of Supervision in STD Control course;
b. Attended the STD Intervention for Supervisors course; and,
c. Attended the Quality Assurance for HIV PCPE course.
Program Managers shall have completed, within one (1) year of becoming a program
manager, all training requirements of a DIS and first-line supervisor.
All staff shall have successfully completed, within six (6) months of employment,
RECEIVING AGENCY - provided Cultural Competency Training course entitled, "The
Never-Ending Journey."
