The Sabine Pass Community Center suffered a broken water pipe in the ceiling. At the time of
the break, all moisture appeared to have been removed from the area. Recently mold appeared in
the ceiling of a portion of the building. Total Safety conducted an inspection survey and
reported there was moisture damage in the ceiling resulting in mold. Total Safety's report
includes what is needed to remediate the area and make repairs. Signature Group of Port
Neches, Texas submitted a proposal through Region 5 Education Service Center Cooperative
Purchasing Organization and has agreed to do the work for $42,442.00 with funding available in
account number 001-15-043-5312-00-20-000-.
ANALYSIS/CONSIDERATION:
The Sabine Pass Community Center is in need of mold remediation and repairs due to a broken
water line in the ceiling. Signature Group of Port Neches, Texas has provided a price for this
work under the Region 5 Cooperative Contract No. 20230404 in the amount of$42,442.00 with
funding available in Account Number 001-15-043-5312-00-20-000-.
RECOMMENDATION
A resolution authorizing the City Manager to enter into an agreement with Signature Group for
mold remediation and repairs at the Sabine Pass Community Center in the amount of$42,442.00.

WHEREAS, the Sabine Pass Community Center suffered a water line break in the
ceiling; and
WHEREAS,mold began to appear in the ceiling; and
WHEREAS, Total Safety conducted an inspection survey of the building and did a
report on what is needed to remediate the mold and make repairs to the building; and
WHEREAS, Signature Group of Port Neches, Texas submitted a proposal through the
Region 5 Education Service Center Cooperative Purchasing Program Contract No. 20230404 in
the total amount of$42,442.00; and
WHEREAS, the Signature Group is qualified to perform the mold remediation and
repairs: and
WHEREAS, pursuant to Chapter 271, Subchapter D, of the Local Government Code, a
city, by resolution of its governing body, may participate in cooperative purchasing programs
established by the State Facilities Commission for the purchase of goods at prices established
through purchase contracts of the Commission; and
WHEREAS, per Resolution No. 20-019, the City entered into an Interlocal Agreement
for cooperative purchasing with the Region 5 Education Service Center Cooperative Purchasing
Program; and
WHEREAS, Signature Group of Port Neches, Texas is a vendor on the Region 5
Education Service Center (ESC) Cooperative Purchasing Program and has provided a proposal
attached hereto as Exhibit"A.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF PORT ARTHUR:

Signature Group (SG) is pleased to have the opportunity to provide the following proposal for mold
remediation and reconstruction of the Sabine Pass Community Building. We are committed to
providing a successful project where safety is our first priority. We look forward to working diligently
towards the fulfillment of this commitment.
Scope of work is as follows:
• Mobilize to site
• Submit a 5-day notice for mold to the State of Texas
• Scope of work will be based off of mold protocol provided by Total Safety
• Hang new sheetrock
• Tape, float, and texture to match existing
• Reconstruction includes prime and paint entire ceiling in damaged areas, not just
affected area
• Painting on walls be corner to corner on each affected area
• Clean work area of all debris

Clarifications and Exclusions:
• This quote is based on a 4-10 work schedule
• Remediation contingency at 25% is $4,664.00
• Reconstruction contingency at 25% is $3,825.00
• Any contingency over 25% will be an additional charge
Pricing includes all labor, tools, and materials per scope. Sales tax will be included at 8.25% unless
a valid resale or exemption certificate is provided by client.
This proposal is made on the assumption that the work will be performed without unforeseen delays
outside the control of Signature Group. If scheduled work is halted for any reason, without cause by
Signature Group, we reserve the right to charge stand-by rates at a minimum of four hours for the
work according to the applicable rates.

Any additional work created by a change in scope could require a Change Order between SG &
client prior to commencement.
If, during the performance of the contract, the price of the material significantly increases, through no
fault of the contractor, the price shall be equitably adjusted by an amount reasonably necessary to
cover any such significant price increases. As used herein, a significant price increase shall mean
any increase in price experienced by contractor from the date of the contract signing. Such price
increases shall be documented through quotes, invoices, or receipts. Where the delivery of material
is delayed, through no fault of the contractor, as a result of the shortage or unavailability of materials
and equipment, contractor shall not be liable for any additional costs or damages associated with
such delay(s).
Terms, fees, and conditions are valid for 15 days from the date of this proposal.

The remediation phase will take approximately 5 days. Total Safety will then re-test and give the clearance (as long as
no additional work is needed) and that usually takes 3-5 days. We will then go in for the re-construction. We are
projecting 8-12 days on reconstruction.
We work a 4-10 schedule, Mon-Thurs.
So I am projecting 16-22 working days.

Limited Mold Assessment
EXECUTIVE SUMMARY
On March 30, 2023, Robert Hanks, Texas Department of Licensing and Regulation Licensed Mold
Assessment Consultant and Sr. EHS Specialist with Total Safety U.S., Inc., performed a limited mold
assessment of the Sabine Pass Community Center in Port Arthur, Texas. The assessment was limited to
the visual observations of the interior spaces, temperature and humidity measurements and collection
and analysis of air samples.This work was requested by Adam Saunders in response to suspected visible
mold.
On November 1, 2017, the Texas Department of Licensing and Registration (TDLR) instituted the Texas
Mold Assessment and Remediation Rules, requiring that all companies and individuals who perform
mold-related activities obtain an appropriate license from the State. A Mold Assessment involves an
inspection of a building to evaluate whether mold growth is present, and to what extent. Samples may
be taken to determine the amount and types of mold that are present; however, sampling is not
necessary in many cases. A Mold Assessment Consultant (MAC) is responsible for developing a mold
remediation protocol that specifies the estimated quantities and locations of materials to be remediated,
the proposed methods to use and clearance criteria that must be met.
OBSERVATIONS
Interviews
• Water damage resulting from the freezing temperatures and plumbing damage occurred in
December of last year and the plumbing repairs have been completed.
• Fire protection crew members reported possible visible mold in the building discovered during
their work.
Visual observations
• The building is of wood construction;the interior finishes of the building includes drywall ceilings
and walls.
• There were no odors noted in the building at the time of the assessment.
• Evidence of water damage and suspect visible mold was noted on the ceiling in the
utility/mechanical room, the restrooms, the rear entrance foyer, the kitchen and in the main
room, primarily around HVAC supply and return registers.
Measurements
• Temperature and humidity measurements were taken in the building as part of the assessment.
Relative Humidity(RH)was 68.9% RH at 69.4°F.
CONCLUSIONS AND RECOMMENDATIONS
1. The interior air sample results indicate an elevation of Aspergillus/Penicillium when compared to
the outdoor samples.These types of spores are primarily allergens and can cause hay-fever like symptoms in
susceptible individuals.
2. The relative humidity in the building was outside the ASHRAE guideline of 20 —60% RH for indoor
environments. The air conditioning should be evaluated by professionals to ensure adequate air
drying and overall efficiency. Consideration should be given to having the HVAC fans, coils, ducts
and registers cleaned or replaced as necessary.
3. As part of a Mold Remediation Project, approximately 200 square feet of drywall and insulation
should be removed from the ceiling in the utility/mechanical room, the restrooms and the rear
entrance foyer. Additionally, approximately 20 square feet of drywall and insulation should be
removed from around the HVAC registers in the kitchen; approximately 20 square feet of drywall
and insulation from around the HVAC registers in the South end of the main room and approximately
16 square feet of drywall and insulation from around the HVAC registers in the North end of the
main room.
4. The building envelope should be inspected regularly to detect water intrusion into interior spaces
and repairs undertaken to prevent fungal growth.
5. Wipe down all built-in furniture items, cabinets,trim and doors in the remediation areas.
6. Apply anti-microbial agent (i.e. Sporicidin or Microban) throughout all remaining surfaces in the
remediation areas.

ASSESSMENT SUMMARY
On March 30, 2023, Robert Hanks, Texas Department of Licensing and Regulation Licensed Mold
Assessment Consultant and Sr. EHS Specialist with Total Safety U.S., Inc., performed a limited
mold assessment of the Sabine Pass Community Center in Port Arthur, Texas. The assessment
was limited to the visual observations of the interior spaces, temperature and humidity
measurements and collection and analysis of air samples.
As a result of the assessment,the following recommendations were made:
1. The interior air sample results indicate an elevation of Aspergillus/Penicillium when compared to
the outdoor samples.These types of spores are primarily allergens and can cause hay-fever like symptoms in
susceptible individuals.
2. The relative humidity in the building was outside the ASHRAE guideline of 20—60% RH for indoor
environments. The air conditioning should be evaluated by professionals to ensure adequate air
drying and overall efficiency.Consideration should be given to having the HVAC fans,coils,ducts and
registers cleaned or replaced as necessary.
3. As part of a Mold Remediation Project, approximately 200 square feet of drywall and insulation
should be removed from the ceiling in the utility/mechanical room, the restrooms and the rear
entrance foyer. Additionally, approximately 20 square feet of drywall and insulation should be
removed from around the HVAC registers in the kitchen; approximately 20 square feet of drywall
and insulation from around the HVAC registers in the South end of the main room and approximately
16 square feet of drywall and insulation from around the HVAC registers in the North end of the
main room.
4. The building envelope should be inspected regularly to detect water intrusion into interior spaces
and repairs undertaken to prevent fungal growth.
5. Wipe down all built-in furniture items, cabinets,trim and doors in the remediation areas.
6. Apply anti-microbial agent (i.e. Sporicidin or Microban) throughout all remaining surfaces in the
remediation areas.

REMEDIATION PROTOCOL - CONTRACTOR
Eliminate Moisture Intrusion
Remediation should be conducted in accordance with the Texas Mold Assessment and Remediation Rules.
Ensure that all moisture intrusion problems in the facility have been identified and completely repaired
prior to beginning remediation work.
Scope of Work:
A licensed mold remediation contractor will be responsible for the following:
1. All items indicated in Table 1,Scope of Work for Remediation Activities.
2. Isolate the work area with a local containment made of six mil polyethylene. Maintain negative
pressure within the containment through the use of a HEPA vacuum or negative air machines.
Cover all HVAC supply and return air registers in the work area with six millimeter polyethylene.
Post warning signs at the entrance to the work area in accordance with OSHA and TDLR
requirements.
3. As part of a qualified Mold Remediation Project,remove all water damaged and/or mold impacted
material as indicated in the Scope of Work. It is estimated that 260 square feet of drywall and
insulation will be impacted throughout the remediation areas.
4. If during the course of remediation additional water damage is discovered, remove any impacted
materials as well. Document the quantity and condition of the materials, and gain approval from
the Consultant/Client prior to removal. The integrity of the wall studs and all other structural
members should be evaluated prior to restoration.
5. Upon completion of gross removal of building materials (as described in Table 1), complete the
following for the building interior in the vicinity of the mold remediation project:
a. Bag all discarded materials in six mil polyethylene bags or taped sheeting. HEPA vacuum
and wipe down bags with a disinfectant solution prior to removing them from the work
area.
b. Wipe down and remove all surface molds from exposed surfaces and wall studs. Wash
affected exposed surfaces and wall studs with an EPA registered disinfectant detergent,
(i.e. Sporicidin or Microban), per manufacturer's recommendations. Ensure that the
material is thoroughly dried to prevent the spread of mold proliferation.

c. HEPA vacuum the floors of the building interior in the vicinity of the work areas. Upon
completion, operate "air scrubbers" equipped with HEPA filtration throughout the area
to clean the air.
d. As part of routine maintenance, inspect, repair and clean the HVAC system prior to re-
occupancy.Clean the HVAC supply and return registers located in the vicinity of the work
areas.
e. Upon completion of remediation, notify the licensed Mold Assessment Consultant so
that a post remediation assessment can be conducted.
f. Upon achieving clearance for all work areas, remove critical barriers.
Personal Protective Equipment
The remediation contractor shall provide appropriate personal protective equipment for executing the
work. Minimum recommendations are as follows:
> Disposable clothing constructed of suitable materials such as DuPont Tyvek or equivalent.
> Negative pressure, half-face air-purifying respirators approved by NIOSH equipped with an N-95
filter.
> Headgear, eye protection, work gloves and footwear of size to properly fit individual workers or
authorized visitors.
Worker Decontamination
Provide a decontamination area to allow workers to properly decontaminate prior to exiting the work
area or removing materials from the work area. Use the following decontamination procedures for
personnel in work area:
> Remove protective clothing before exiting the work area.
> Wet clean or HEPA-vacuum respirator, inner suit and exposed portions of the body in
decontamination area.
> Maintain respiratory protection throughout the decontamination process.

POST REMEDIATION CLEARANCE REPORT
After remediation is complete for each work area, air scrubbers should be operated for at least 24 hours
prior to the post remediation assessment. The post remediation assessment shall be conducted prior to
build back.
The work area must be free from all visible mold, wood rot and water damage. Spore trap air samples
and/or surface samples will be collected while the containment structure is still erected to determine
adequate remediation.
The Mold Assessment Consultant will perform a post remediation clearance including the following:
Visual inspection -A visual inspection of remediated areas for the absence of visible mold, dust,
and wood rot;
Analytical evaluation — air samples and/or surface samples, as determined by the Mold
Assessment Consultant
A Post Remediation Clearance report will be issued by the Mold Assessment Consultant upon meeting
clearance criteria.

Any additional work created by a change in scope could require a Change Order between SG &
client prior to commencement.
If, during the performance of the contract, the price of the material significantly increases, through no
fault of the contractor,the price shall be equitably adjusted by an amount reasonably necessary to
cover any such significant price increases. As used herein, a significant price increase shall mean
any increase in price experienced by contractor from the date of the contract signing. Such price
increases shall be documented through quotes, invoices, or receipts. Where the delivery of material
is delayed, through no fault of the contractor, as a result of the shortage or unavailability of materials
and equipment, contractor shall not be liable for any additional costs or damages associated with
such delay(s).
Terms, fees, and conditions are valid for 15 days from the date of this proposal.

So I am projecting 16-22 working days.
