COMMENTS FOR * REQUESTED BY: Ernestine Wade, RN, Director ADMINISTRATION * PURPOSE EXPRESSED BY REQUESTOR: · Approval of a contract between the City USE * of Port Arthur and Rose Quirante, R.D. for professional · dietary consultation services ONLY * · LOCATION: NIA RECOMMENDATION: It is recommended that the City Council approve Proposed Resolution No. 13281 approving a contract between the City of Port Arthur and Rose Quirante, R.D., a licensed registered dietitian for the purpose of providing professional dietary consultation services for the WIC Program and diabetes education classes to the citizens of Port Arthur. BACKGROUND: Mrs. Quirante presently contracts with the WIC program to provide weekly dietary consultation services for its clients. Some of her present duties include assisting with the quality assurance program, providing in-service training on nutrition topics related to women, infants and children; assisting with the nutrition education and breasffeeding plans, providing individual counseling for participants with high-risk conditions, and conducting nutrition education classes. Mrs. Quirante provides these services to WIC for no more than 32 hours per month. In that the Health Department has expanded its present program services by offering weekly diabetes education classes to the citizens of Port Arthur, Mrs. Quirante has agreed to continue to provide these additional services for no more than 1 hour per week. STAFFING/EMPLOYEE EFFECT: It is recommended that Mrs. Quirante remain as a contractor for the WIC program and be allowed to provide additional diabetes education classes on a weekly basis. BUDGET EFFECT: Funds to pay for contractual services are being provided through the Bureau of Nutrition Services (WIC) at a rate of $40 per hour. The additional diabetes education classes will be funded through the Preventive Health grant at a rate of $45 per hour. No. 13281, as such contract allows for Mrs. Rose Quirante to continuing providing dietary consultations for both the WIC and diabetes education programs for the citizens of Port Arthur. QUlRANTE, R.D., FOR THE PROVISION OF DIETARY CONSULTATION SERVICES FOR THE WOMEN, INFANTS AND CHILDREN 0/VIC) PROGRAM AND DIABETES EDUCATION CLASSES TO THE HEALTH DEPARTMENT CLIENTS WHEREAS, the Bureau of Nutrition Services has recommended that the Port Arthur City Health Department WIC Program contract with a registered dietitian to provide consultation services to the program's participants; and, WHEREAS, the Port Arthur City Health Department also wishes to contract with a registered dietician to provide diabetes education to the citizens of Port Arthur; and, WHEREAS, Rose E. Quirante, R.D., is licensed to practice as a Registered Dietitian Consultant and has agreed to provide professional consultation services for the WIC program at a rate of $40 per hour, not to exceed 32 hours per month, and at a rate of $45 per hour for the diabetes program, not to exceed 4 hours per month; now therefore, BE IT RESOLVED BY THE CITY COUNCIL OF PORT ARTHUR: THAT the City Manager and Director of Health are hereby authorized to execute said Contract, a copy of which being attached hereto as Exhibit, "A"; and, THAT said Contract is an exact duplicate of a City Standard Form Contract except for the changes described in the Certificate of Standard Form Contract, a copy of which P.R. 13281 Page 2 is attached hereto as Exhibit "B" and is hereby incorporated by reference, and said changes are hereby approved; THAT a copy of the caption of this Resolution be spread upon the Minutes of the City Council. Quirante, R.D., a person licensed to practice as a Registered Dietitian Consultant. I. TERM OF AGREEMENT That this Agreement shall be for a period beginning October 1, 2005 shall remain in force through September 30, 2006. Thereafter, this Agreement shall be renewed annually at the discretion of the parties. I1. PROFESSIONAL SERVICES That Mrs. Rose E. Quirante, R.D., will be considered an independent contractor for the City of Port Arthur and shall provide dietitian consultation for the Women, Infants and Children (WIC) Program at the City's Health Department. She shall also provide diabetes education services in accordance with the Department of State Health Services Diabetes Council. Such services shall be at the standard of care/knowledge expected of dietitians in Texas. III. COMPENSATION That Mrs. Rose E. Quirante, R.D., shall be paid Forty Dollars ($40.00) per hour for nutrition dietary consultations for the WIC program, and she shall be paid Forty-Five Dollars ($45.00) per hour for nutrition dietary consultations for the diabetes program. The hours of work shall be mutually agreed upon by the Health Director of the City of Port Arthur and Mrs. Rose E. Quirante, R.D., and shall not exceed 36 hours per month. IV. LICENSURE That Rose E. Ouirante, R.D., shall maintain and provide proof of licensure with the State Board of Registered Dietitians. Failure to maintain licensure will immediately render this contract null and void. V. BENEFITS That ali benefits resultant from this contract are delineated herein, and no other benefits promised by any City office shall be binding on the City. VI.
